Document Confirmation

Before you proceed further, make sure you have all these required documents:

1. Your recently taken Photo (Supported file types: jpg, jpeg, png, pdf)

2. Copy(s) of academic certificate(s) (Supported: jpg, jpeg, png, pdf)

3. Recommendation Form/Letter (Supported: jpg, jpeg, png, pdf)

4. Personal Testimony (Supported: jpg, jpeg, png, pdf)

Click here to download the  Recommendation form. Print it (A4 size paper) and It should be filled and signed by either your Pastor or Leader. Once filled, scan it or take a picture of it. Then you should be ready to go.
